Community Link Worker
The Health Agency currently employs a Community Link Worker (CLW) as a joint partnership with Edinburgh Health and Social Care Partnership funded by the Scottish Government. Our Link Worker, Julie Roxburgh, is based full time at Wester Hailes Medical Practice, accepting referrals from staff within the practice for registered patients aged 16 and over.
Our CLW is a social practitioner offering non-clinical support to Wester Hailes patients via social prescribing. By working together over several sessions, the CLW supports you to set goals and overcome barriers so that you can take greater control of your health and wellbeing. Our CLW helps people to achieve their goals by supporting them to identify and access relevant resources and services within their community, following a referral from staff within Wester Hailes Medical Practice.
